If You Believe We Fell Short

If you believe we failed to provide these services, or discriminated on any basis listed above, file a grievance with our Civil Rights Coordinator in person, by mail, by phone, or by email. If you need help filing, the Coordinator will help you. The process is simple:

• File your grievance within 60 days of the event.
• We investigate. Expect us to contact you if we need more detail.
• You receive our written response within 30 days.

Filing With the Federal Government

You can also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at any time within 180 days of the alleged discrimination:
