A forest disappearing into morning fog

Brain fog

Can’t think straight? Let’s find out why.

Forgetting words, rereading the same page, running on empty by mid-afternoon. Brain fog is a symptom with causes, and most of them show up on a careful history and a blood test.

Sound familiar?

You’re not imagining it.

  • You read the same paragraph three times.
  • The word is on the tip of your tongue, and it stays there.
  • You walk into a room and forget why.
  • Afternoons feel like wading through mud.
  • You’re tired even after a full night in bed.
  • Your focus at work isn’t what it was.

Thinking runs on sleep, hormones, oxygen and fuel. When one is short, focus is often the first thing to go.

What’s happening in your body

Fog has causes. Most can be checked.

Six common, checkable contributors. Often more than one is at work, which is why guessing rarely fixes it.

Brain fog Sleep Hormones Thyroid Iron B12 Bloodsugar
  1. SleepToo little, or broken by night sweats, snoring or stress, and the next day thinks slowly.
  2. HormonesPerimenopause and menopause in women, low testosterone in men.
  3. ThyroidAn underactive thyroid slows everything, thinking included. A blood test finds it.
  4. IronLow iron stores (ferritin) can cause fatigue even when your blood count is normal.
  5. Vitamin B12Low B12 can affect memory and nerves. A blood test finds it.
  6. Blood sugarHigh or swinging blood sugar can sap energy and focus. An A1c test checks it.

Questions

Common questions

Is brain fog a real medical problem?

It is a description, not a diagnosis, and that is exactly why it deserves a workup. Many of its causes, including thyroid, iron, vitamin B12, blood sugar, sleep and hormones, can be found with a careful history and blood tests.

Which tests will I have?

Your physician chooses with you. Usually a complete blood count, ferritin, thyroid tests and vitamin B12, and, where they fit, hormone levels and blood sugar.

Is it early dementia?

In midlife it rarely is. Studies of women through the menopause transition find small memory changes that, for all but a very small number, stay within the normal range. New or worsening confusion should still be checked, and sudden confusion, weakness or trouble speaking is an emergency: call 911.

What does it cost to start?
